Instructions for New Patients on Completing and Submitting Intake Forms

Welcome to our clinic! To get started as a new patient, please follow the steps below to complete and submit your intake forms. This process is crucial for us to provide you with the best care possible.

  • Obtain the Necessary Intake Forms: Your first step is to acquire a copy of our intake forms provided below, which includes the following documents:

Form A (Policy Form): This is a mandatory form for all new patients. Please make sure to fill in the name of your designated doctor (DR.Saad Alam or Dr.Mona Nasiri) in the space provided on the first page.

Form B (Adult History Form): If you are 18 years old or older, filling out this form is required.

OR

Form C (Pediatric History Form): If the applicant is under 18 years old, this form should be completed by their parents or legal guardians.

Transfer of Records Form: This form is only necessary if you have a previous family doctor or have been a client at another clinic. If this is not applicable to you, you can skip this form.

Please Note: If you are unable to open the forms on your computer using Adobe Reader or any other PDF reader, please try opening them with Google Chrome or Mozilla Firefox. After completing the forms, you can download them for submission.

  • Completing the Forms:
    • Carefully fill out each form that applies to you. It is essential that adults complete both History Form and Policy Form.
  • Submit the Completed Forms:
    • Once you have filled out the necessary forms, please email them to intake@divinemedicalclinic.ca and write New Patient Request -Name of Doctor for email subject.For example Subject: New patient Request-Dr.Alam
    • Attach a Copy of Your Care Card: For identification purposes, please include both the front and back copies of your care card. If you have concerns about sending your care card information via email, you are welcome to bring a copy to the clinic later.
  • What Happens Next?
    • After we receive your completed forms, we will begin processing your application. Please be aware that this process can take between 2 weeks to 2 months, depending on doctor availability.
    • We will contact you once your application has been processed and provide you with further instructions or schedule your first appointment.

Due to a high volume of calls, we kindly request that you refrain from calling regarding this matter.
